1. Office Credits last 90 days. Not including Gift Cards.
2. No returns. Only Product for product, and service for service of same or equal value.
3. Original receipt must be present at the time of exchange.
4. A card must be on file to book appointments.
5. Pre-purchased services must be used within the year of purchase.
6. No exchange or credit can be given in replacement of service or treatment.
7. All NO-SHOW appointments will be charged $50. Client will not be able to reschedule until the no-show fee is paid in full.
8. If you must cancel or reschedule any appointment, please provide us with 24-hour notice.
9. Clients will be allowed a 15-minute grace period. If unable to arrive on time, you may have to forgo parts of the service to keep it in the time allotted for you. Please, ALWAYS CALL if you even think you might be late.
10. In-Office Consultation Fee – $50: A standard fee of $50 applies to all in-person consultations. This covers the time spent reviewing your concerns, discussing treatment options, and creating a personalized care plan during your visit.
11. Prescription Management Fee – $50: A $50 fee is required for prescription requests that are not addressed during a scheduled consultation. This requires a separate appointment and does not include services related to Bioidentical Hormone Replacement Therapy (BHRT).
12. Video Consultation Fee – $100: Virtual consultations are available for $100. This fee includes the convenience of remote access to care, allowing you to consult with your provider from the comfort of your home while still receiving personalized medical guidance.
